It should be in the distribution you fetched in the FORMS
subdirectory. A

make install

should've installed it for you.

Again, it sounds as if you have fetched the wrong version of the
library dsidtribution for your operating system. Did you fetch the
ELF version of the libarary or the a.out version of the library? The
a.out version of the library does *not* contain the shared library,
the ELF does.

Look in

ftp://einstein.phys.uwm.edu/pub/xforms/linux/elf/bxform-088.tgz

No, it's something different. It may be involved with the old curses
ASCII display management tools.
