XForms: fdesign includes xbm or xpm files more than once

It appears that there is a bug in the handling of xpm or xbm include files
for Pixmap or Bitmap objects: if you include the same image multiple times
in a form, `fdesign' generates multiple #include statements. This means
that unless either the form's .c file is edited or the xpm or xbm files are
modified to avoid multiple instantiations of variables, a compiler error
is generated.
